Boat Slip Application

This form is the first step if you would like to be considered for an Elks Boat Slip.

Boat Slip Rules & Indemnification Form

This form is required to be submitted every year.

Boat Slip Hours Form

This is where you list Volunteer hours you did for the year. The year begins each November 1st and ends October 31st. Failure to submit a properly completed form will result in forfeiture of your boat slip or place on the wait list.
